This privacy policy, as part of the consent form, sets out how The Good Gut Dietitian uses and protects any personal information that you provide us.

The Good Gut Dietitian is committed to ensuring that your privacy is protected. Should we ask you to provide certain information by which you can be identified when using our services, you can be assured that it will only be used in accordance with this privacy policy.   

This policy is effective as from 09/10/2020.

Why we collect your data: 

We collect your personal data because you give us consent to do so, in order to provide our independent nutrition and dietetic services to you.

What we collect: 

We may collect the following information: 

  • Information in relation to this consent form including full name, occupation, gender and marital status
  • Contact information and personal details including home address, telephone number, email, details of GP and other medical professionals
  • Further clinical information in relation to reports from other health professionals including current and past medical history, body measurements and lifestyle information provided by the client

What we do with the information we gather: 

We require this information for the purpose of: 

  • Professional clinical record keeping of client information and sharing information with relevant health professionals
  • To provide a service to you and deal with your enquiry if you complete our contact form

Security 

We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place encrypted electronic systems and standard operating procedures to safeguard and secure the information we collect. This includes:

  • Paper records containing personal data is kept in an locked cabinets and only accessed by the Good Gut Dietitian
  • Laptops are password protected

Controlling your personal information 

We will not distribute, sell or lease your personal information to third parties unless we have your explicit permission or are required by law to do so.   

You may request details of personal information which we hold about you under the General Data Protection Regulation (GDPR).  If you would like a copy of the information held on you please contact Mrs Sadia Latimer Data Protection Officer, at this address The Good Gut Dietitian, PO Box 951 WA15 5RP.

If you believe that any information we are holding on you is incorrect or incomplete, please email or write to us as soon as possible at the above address. We will promptly correct any information found to be incorrect.   

Cookies

If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.

When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. If you select "Remember Me", your login will persist for two weeks. If you log out of your account, the login cookies will be removed.
